Risk Assessment Scale
0–4 points: Low Risk. There may be signs of unhealthy behaviour, but immediate physical danger or escalation is not strongly indicated. Caution and self-monitoring are still advised.
5–9 points: Medium Risk. Multiple warning signs of abuse and control are present. The situation may escalate. Support and a safety plan should be seriously considered.
10–15 points: High Risk. There is a significant risk of harm or violence. Urgent action is advised. Consider contacting a support organization, shelter, or law enforcement for protection.
